FINASTERIDE
Drug Facts:
Mechanism of Action
Finasteride is an inhibitor of 5-alpha reductase, an enzyme that converts testosterone to dihydrotestosterone (DHT).This enzyme is located on the surface of the prostate and is responsible for the production of DHT. DHT is a derivative of testosterone that is converted to DHT by the enzyme 5-alpha reductase. DHT is the primary androgen in the body that is responsible for the male sexual characteristics and is also found in the female sex hormone binding globulin (SHGB) and in the male sex hormone levels. When the production of DHT by the 5-alpha reductase is decreased, there is an increase in the production of DHT and the symptoms of benign prostate hypertrophy (BPH) can also be treated with Finasteride.
Finasteride inhibits the action of Type II 5-alpha reductasein prostate cells. This enzyme is located on the surface of the prostate and it is responsible for the production of DHT. DHT is the primary androgen that is found in the female sex hormone levels. In women, the production of DHT by 5-alpha reductase is decreased by around 50% when compared with the adult female sex hormone levels (about 80% of the body of the female sex hormone).
DHT is the primary androgen that is found in the male sex hormone levels. In men, the production of DHT by Type II 5-alpha reductase is decreased by around 50% when compared with the adult female sex hormone levels (about 80% of the body of the female sex hormone).
Finasteride reduces the levels of prostate specific antigen (PSA) in menthat have BPH. This prostate specific antigen is found in the prostate tissue. Finasteride reduces the level of PSA in the prostate.

The average cost of generic Propecia fell for the first time this year, from a year ago, but many consumers aren’t looking at the hefty price tag and are instead waiting for the generic version to hit the market.
Last year, the price of generic Propecia dropped to just under $8 per tablet from $2.50 per tablet last fall. By comparison, the price of the brand-name drug Proscar has risen to just under $14 per tablet since then.
The price reflects a broader trend of lower cost generic versions of branded drugs going for cheaper prices, says Dr. Elizabeth Kavoussi, director of the FDA’s Center for Drug Evaluation and Research. “You have the generics now and the brand-name versions. There’s a wider market,” she says.
Kavoussi notes that generic Propecia has been approved for people with mild to moderate hair loss, including male pattern baldness. The drug’s price has also fallen, from $1.50 to $4.30 per tablet, depending on the dosage.
“There’s not a lot of competition,” she says. “That’s why generic Propecia has been so expensive.”
Despite the price reduction, some consumers are also looking for ways to cut the cost of the generic version of the drug, Kavoussi says. For instance, generic versions of the hair loss drug Minoxidil are available in a lower price range, Kavoussi says.
The generic version of finasteride, sold under the brand name Propecia, can be taken daily in pill form. A higher strength tablet, a month’s supply of the drug, is available in a lower price range.
In some cases, generics may not be available in the generic version, Kavoussi says. “If you can’t get the generic version, you can’t get the brand-name drug,” she says.
Generic versions of the hair loss drug Dutasteride can be taken daily in pill form.
Generic versions of the hair loss drug Propecia also can be taken daily.
As of this writing, the average cost for generic Propecia has risen from $2.50 to $8.15 per tablet, to $2.70 per tablet last fall. The cost of generic Proscar has risen from $3.05 to $12.40 per tablet from $1.70 to $12.40 last fall.
While it is true that generic Propecia will likely be the most expensive drug on the market, Kavoussi cautions that generic Propecia, as well as its brand-name counterpart Proscar, is not without its competition.
For example, if a patient is prescribed a generic version of a hair loss drug that will cost between $8.15 and $12.40 per tablet, they should consult with their doctor before switching to the generic version.
Some consumers who are considering switching to a generic version of their drug, Kavoussi says, can also consider taking a lower strength version of the drug. A month’s supply of the drug might be sufficient.
For those patients, the average cost of generic Propecia will be around $6.10 per tablet.
While Kavoussi cautions that generics aren’t as expensive as brand-name drugs, they may be a good option for many patients who don’t want to pay more for their brand-name versions.
The cost of generic versions of branded drugs also has declined, Kavoussi says. For example, in May, generic versions of the hair loss drug finasteride, sold under the brand name Propecia, cost $1.60 per tablet. The price could be as high as $12.40 per tablet, depending on how much the patient has to save.
However, many consumers don’t see generic versions of drugs as an option for hair loss treatment. “If you can’t get the generic version, you can’t get the brand-name drug,” says Kavoussi, adding that most people don’t really know whether a generic version of their hair loss drug is available for them.
